Directions for Using the Form

Notice there are drop down boxes associated with several of the data fields. Please use them to designate which address or addresses are being modified. For example, to the left of the field for "street address" is a box with the default selection "Home". From the dropdown arrow you may also choose "Office, Home #2, College, WorkSite" and so on. Another drop down box tells us what you want done with the address: "Please Do Not Change It", "Replace it with the one Below", or "Discard It". Please make sure the appropriate options are selected for each data field you use.

Please indicate your primary preference for being contacted (Email, Postal mail, Telephone, or Cell phone) by choosing the appropriate option from the drop down menu under "Contact Preference". Even if you choose a method other than "Postal mail" it is still helpful for us to have your postal mail address. Phone numbers and email addresses tend to change with no forwarding address for us to keep up with you!

When you finish filling out the form, click on the "Submit Form" button at the bottom. You may then close the form page on your browser and return to the Hard Hats website.
